I am a 38-year-old male with pelvic and testicle pain, which has come intermittently since 5 months ago. This actually happened after I played kabaddi (Indian rugby). The opponents actually fell on me. The pain is localized on my right pelvic bone and left pelvic bone. I did go to see several doctors. They prescribed pain killers, checked me physically, and said they couldn’t find anything there, including hernia. I went to see a urologist, and he too checked me thoroughly and said I’m fine. He did a scrotum ultrasound and said all appears to be normal. The pain became consistent, and I went and saw another urologist who did a physical check at the pelvic and testicle area. While my pelvic region is fine, he said he could feel enlarged veins, and it could be epididymitis. He, therefore, asked me to do another scrotum ultrasound. While doing the ultrasound, they asked me to breath out heavily and found a grade 1 varicocele on both sides of the testicle. The doctor then said surgery is not necessary.
But I notice the left side pelvic pain and left testicle pain are more prominent than on the right side. When I apply pressure on my left pelvic bone, it appears to be really painful compared to the right side. So far, I did not notice and lump or anything like that. I feel like an old untreated wound when I try to massage that place. I read over the net that pelvic pain can cause varicocele, and once the pelvic muscle or floor is realigned, we can reverse the problem. Can you help? May I know what treatment is available and roughly how much will be the cost?
Just for your info, I have gastritis, and back in May 2015, I have a complete pelvis CT SCAN which comes back normal.

Pain in the pelvis and testicle that began after a crushing load in kabaddi, with several doctors finding nothing structural and hernia excluded, usually means the examination has been directed at where the pain is felt rather than at what refers pain to that area. Those are frequently not the same place.
One thing has to be settled first. Testicular pain requires the scrotal causes to be excluded properly, and a physical examination alone does not do that. If you have not had a scrotal ultrasound and a urology review, arrange those before pursuing a musculoskeletal explanation, because varicocele, epididymal, and other local causes are common and are missed on palpation. Sudden severe testicular pain, swelling, fever, or blood in the urine is a same-day medical matter, not something to investigate slowly.
Once those are cleared, three musculoskeletal sources refer into exactly the region you describe, and a direct blow to the pelvis can start any of them.
The pubic symphysis and the adductor origin. A crush injury loads the front of the pelvic ring, and pain there is felt across both pubic bones and often into the groin and scrotum. It is reproduced by resisted squeezing of the knees together and by direct pressure over the joint.
The sacroiliac joints. Trauma through the pelvis commonly leaves one of these irritated, and it refers into the groin as readily as into the buttock. Standard lumbar imaging does not show it, which is why it survives several normal scans.
The nerves that supply the groin and scrotal skin. The ilioinguinal, iliohypogastric, and genitofemoral nerves arise around T12 to L1 and can be irritated either where they pass through the abdominal wall or at the thoracolumbar junction itself. This referral pattern is regularly missed because the painful area sits a long way from the segment producing it, and because the lower back may not hurt at all.
What an examination should cover, then, is direct palpation of the pubic symphysis, resisted adductor testing, sacroiliac provocation testing, segmental examination of the thoracolumbar junction, and sensory testing across the groin. Intermittent pain across five months with a clear traumatic onset is a pattern worth examining that way rather than repeating imaging that has already come back clear.
