Why Combine the Two Rather Than Choose Between Them
Patients often ask which they need, a chiropractor or a physiotherapist. The more useful question is what each one reaches, because the answer explains why we do not separate them.
Manual therapy has been measured against the alternatives. A systematic review and meta-analysis of randomized trials found that spinal manipulative therapy produces effects similar to the other therapies clinical guidelines recommend for chronic low back pain, with adverse events that were mostly musculoskeletal, transient, and mild to moderate (Rubinstein SM, et al. BMJ. 2019;364:l689. doi:10.1136/bmj.l689).
Similar to, not better than. That is the honest reading, and it is the reason manual care on its own is an incomplete plan. Restoring movement at a restricted segment does not strengthen the muscles that stopped protecting it.
Exercise covers that gap, and it has its own evidence. A 2021 Cochrane review found exercise probably reduces pain in chronic low back pain compared with no treatment, usual care or placebo, with minor adverse effects (Hayden JA, et al. Cochrane Database of Systematic Reviews. 2021;CD009790). Guidelines place both in the first line rather than in sequence (Qaseem A, Wilt TJ, McLean RM, Forciea MA. Annals of Internal Medicine. 2017;166(7):514-530. doi:10.7326/M16-2367).
So the case for combining them is not that two treatments are better than one. It is that they do different jobs. The chiropractor restores movement where a segment has stiffened; the physiotherapist works on the muscle, the soft tissue and the loading that allowed it to stiffen. Done in separate clinics on separate days, those two halves rarely get coordinated. Done in one session, they can be.
If a plan you are offered contains only passive treatment, with no exercise you take home and no change to how you sit, lift or sleep, that is the half that tends to be missing.
