Chiropractic in Malaysia: How the Methods Differ
Chiropractic in Malaysia is practiced in very different ways from one clinic to the next, so the method matters as much as the word on the signboard. Some chiropractors use manual techniques such as Gonstead and Diversified, which are well established and good for mobilizing joints but usually twist the spine and produce a cracking sound. Others, as I do for most patients, use methods without twisting: the Activator instrument, SOT, flexion-distraction and the Thompson drop table. At Chiropractic Specialty Center (CSC), which I founded in Kuala Lumpur on 26 December 2006, chiropractic is planned with registered physiotherapy from one examination, because an adjustment alone cannot reach every tissue at depth. The method follows the examination: a stiff joint, an irritated nerve root and a weak muscle each need something different.

Chiropractic in Malaysia: Key Takeaways
- Chiropractic is safe when it is performed by a competent chiropractor who is also registered.
- Gonstead and Diversified suit younger spines without degeneration or disc damage.
- With moderate or severe degeneration, disc problems or nerve compression, care is given without twisting the spine.
- Do not choose between a chiropractor and a physiotherapist; choose both.
- The examination is RM100 and an adjustment RM150, or from RM110 when the visit includes physiotherapy.
How Chiropractic in Malaysia Is Regulated
Chiropractic is registered in Malaysia under the Traditional and Complementary Medicine Act 2016 (Act 775), and registration follows a university degree program. Someone who cracks bones without that formal education is offering bare manipulation, not chiropractic. Training differs too: the Doctor of Chiropractic is a professional doctoral degree, and I hold mine from Cleveland University-Kansas City (1996). Before a first visit anywhere, ask whether the chiropractor is registered.
Manual Techniques: Gonstead and Diversified
There are hundreds of chiropractic techniques. Gonstead and Diversified are two of the most common: they are well established and great for mobilizing joints, and they usually involve a twist and an audible release. In a young person without degeneration or disc damage, they are fine. How the two compare with the Activator is explained in Gonstead, Diversified and Activator.
Booking Chiropractic in Malaysia at Two KL Centers
New patients can usually be seen the same day at either Kuala Lumpur center, but call or send a WhatsApp message before you travel so a chiropractor is free and your wait is short. The first visit is an examination: your history, posture, how each joint and muscle moves and, where symptoms reach an arm or leg, the nerves, before any method is chosen. Hours and directions are on the contact page.

Non-Rotatory Methods: Activator, SOT, Flexion-Distraction, Thompson
When a spine has moderate or severe degeneration, a damaged disc or a compressed nerve, I use care that does not twist it, because more than that may inflame the area or worsen the disc damage. The Activator delivers a light, quick impulse to one point; SOT uses padded wedges under the pelvis; flexion-distraction gently opens and closes the spinal segments on a moving table; the Thompson drop table lets a section of the table drop a short distance so the thrust is small.
In this video I answer the thirteen questions I am asked most, from who may practice to what the cracking sound is. The chapters and transcript are on the Is Chiropractic Safe? video page.
My two-minute video shows the contrast on a patient. I demonstrate both on a patient: first the non-rotatory method with the Activator, direct contact on the segment with no twisting and no cracking, then the rotatory side posture, where the spine is tilted and rotated, the patient is rolled toward the chiropractor, and a thrust may produce a crack.
The transcript and key moments are on the Rotatory vs Non-Rotatory Adjustment video page.
What the Cracking Sound Is
The sound is cavitation: gases released from the fluid inside the joint, as when you crack a knuckle. It is not a bone going back into place, and it is not a measure of whether the adjustment worked. For people in their forties and older with degenerative changes, bone spurs or a pinched nerve, I would not recommend a method that produces an audible; the Activator is the better choice there.
Chiropractic in Malaysia With Physiotherapy
An adjustment restores movement to a joint. It does not strengthen the muscles that failed to protect that joint, and it does not reach every muscle, ligament and tendon at depth; registered physiotherapy does. So do not choose between one or the other; choose both. Holistic care means comprehensive care, not only natural care.
Exercise Rehabilitation Is Part of the Plan
Every plan includes exercise chosen for the condition and the age, with step-by-step instructions, such as the back exercises guide. Exercise starts when the body is ready for it, and it continues after the pain settles.
Spinal Decompression and NSD Therapy®
Spinal decompression is a non-surgical option for disc-related problems, used when the examination supports it. NSD Therapy® is the name I gave to the combination of spinal decompression, chiropractic, clinical physiotherapy and exercise rehabilitation that CSC uses for non-operative spine care, including a slipped disc, sciatica, spondylolisthesis and spondylosis.
When Is an X-ray or MRI Needed?
Not every patient needs imaging. Ordering an X-ray before assessing the patient is not competent care, so please do not have one taken before you come. Many patients are cared for without any imaging at all; that is decided case by case. Imaging comes after a thorough assessment and only when it is indicated: sectional X-rays for the neck or back, full-spine X-rays only for scoliosis. An MRI is the gold standard, with zero radiation, and it shows bone, joint, muscle, ligament and the space around the nerves; an X-ray is cheaper and shows bone only.
How Progress Is Reviewed
Progress depends on how severe the problem is, any complications, and how closely the advice is followed. If there is no improvement by the 5th session, the case is reassessed. If there is still none by the 10th, we discuss whether to stop or continue. Care should never be uncomfortable; my motto has always been no pain, more gain. Chiropractic should have a purpose and a goal, set by what the examination finds. I do not recommend regular adjustments for people with no symptoms to "boost" the body's innate intelligence or immunity, just as a daily aspirin makes sense only for someone with an underlying vascular problem. If it is not broken, do not fix it; once it has been fixed, a maintenance or preventive visit now and then, for someone who has had episodes of neck, back or joint trouble before, keeps the repair from going bad. That is the only kind of wellness plan I recommend.
Signs That Need a Hospital
Wherever in Malaysia you receive chiropractic care for back or leg pain, new numbness around the genitals, between the genitals and anus or around the anus, new difficulty passing urine, or any loss of bladder or bowel control needs a thorough assessment at a hospital emergency department straight away, within hours. Leg weakness that keeps getting worse should not be ignored. It calls for a careful assessment by a competent provider, and conservative care can continue only while the weakness itself is improving.
Choosing Chiropractic in Malaysia: What to Check
- Is the chiropractor registered, and what is the qualification?
- How will the problem be examined before any method is used?
- Which method will be used, and does it twist the neck or back?
- Are physiotherapy and exercise part of the plan?
- How will progress be measured, and when is it reviewed?
A second opinion is reasonable whenever you are unsure, including before spine surgery, which should be the last option after the conservative route has been exhausted.
What the Research Says About Spinal Manipulation
A review of randomized trials found that spinal manipulation for chronic low back pain gives results similar to other recommended treatments, and that most side effects were temporary and mild to moderate [1]. A population study of vertebrobasilar stroke in Ontario found the small rise in risk after a visit was similar for chiropractors and family doctors, most likely because people with an early artery tear seek care for neck pain and headache before the stroke [2]. A broad evidence report reviewed manual therapies across many conditions [3].
What Does Chiropractic in Malaysia Cost at CSC?
Fees are the same at every CSC center. The quotation dated June 25, 2026 lists the examination at RM100, a chiropractic adjustment at RM150, or from RM110 for the adjustment when the visit also includes physiotherapy, flexion-distraction at RM45, or RM35 with physiotherapy, and the reading of an MRI or X-ray you bring at RM50. Each item is on the chiropractic price and physiotherapy fees pages. The fee for a plan is explained and agreed before care begins.
