What Causes Backache
Backache is usually a problem of load. The joints, discs, muscles and ligaments of the back all share the work, and pain often starts when one part carries too much for too long. Common causes include:
- Sitting or standing in one position for long periods
- Long drives
- Lifting the wrong way, lifting often, or lifting heavy loads
- Bending forward over a phone or screen for hours
- Weak or tired back and stomach muscles
Sitting at work and then sitting again at home adds up. Short movement breaks through the day cut those long spells of sitting into smaller ones.

What the Evidence Says About Recurring Backache
The Lancet’s low back pain series pointed out that first-line care is used too little, while scans, bed rest, strong painkillers, spinal injections and surgery are used too much (Foster 2018).
A Cochrane review found that exercise probably eases long-term low back pain more than no treatment or usual care, with only minor side effects (Hayden 2021). Spinal manipulation works about as well as other recommended treatments for long-term low back pain (Rubinstein 2019), and reviews support manual therapy as a reasonable option (Bronfort 2010).
Some long-term back pain is linked to the way the spine is moved and controlled, which is why movement retraining is part of care (O’Sullivan 2005). Some people also find that moving in one direction eases their pain, an idea used in the McKenzie method (McKenzie 1981).
So a backache that keeps coming back is rarely fixed by treatment alone. Changing the load gives the back a better chance of fewer episodes: how you lift, how long you sit without moving, how you sleep, and whether the muscles around the spine are doing their share.
Simple Ways to Protect Your Back
- Take short movement breaks often when sitting.
- Keep screens at eye level, and hold your phone up rather than looking down for long periods.
- Lift with the load close to your body, and avoid twisting while lifting.
- Stay active. Walking is a simple, low-cost start.
- Keep up your home exercises, even after the pain settles.
When to See a Doctor First
Some back pain should be checked by a doctor before chiropractic or physiotherapy, such as pain after a bad fall, pain with fever, or pain in someone with a history of cancer. Weakness in a leg that keeps getting worse should be checked promptly by a competent provider, and careful non-surgical care may still be a reasonable route afterward. If you have backache, with or without leg pain, and develop new or worsening numbness around your genitals, between your genitals and anus, or around your anus, get urgent medical attention: a thorough assessment by a competent clinician the same day. The same applies to new difficulty passing urine, even if you have not lost bladder or bowel control, and to any loss of bladder or bowel control. These symptoms can have several causes, and an assessment helps identify the right care. Some causes may call for an invasive procedure, but being assessed does not mean you need surgery.
