      Yes, though the term itself is worth unpacking, because it causes more worry than it should.

      Degenerative disc disease is not a disease in the usual sense and it is not progressive in the way the name suggests. It is a description of discs that have lost water content and height with age. It is present on the imaging of most adults past middle age and on a substantial share of people in their thirties, including large numbers with no back pain at all. Being given the label tells you what your scan looks like. It does not tell you why you hurt.

      That distinction determines what is actually treated. Nobody reverses disc degeneration, and any practitioner claiming to regenerate a disc is overstating what is possible. What can be addressed are the consequences that produce symptoms, and there are three.

      Loss of disc height pushes the small joints at the back of the spine together, so they carry load they were not designed for. That produces pain worse on standing and leaning back, and it responds to restoring segmental movement and to reducing sustained extension.

      Reduced height also narrows the opening the nerve root exits through, which is why degenerative change sometimes produces symptoms down a limb. Where a root is involved, care is directed at reducing its irritation and at the positions that close that opening further.

      And the muscles that stabilize the segment weaken, both with age and with any period of pain-limited movement. This is the part most often left untreated, and it is the most correctable of the three.

      So what a plan should look like is an examination that establishes which of those is producing your symptoms, treatment aimed at that mechanism, and a progression into strengthening rather than an indefinite course of the same session. Care that stops when the pain settles is the most common reason people return with the same problem a year later.

      Some have found spinal decompression therapy by devices like the RxDecom or similar to be helpful for spinal disc related issues, including those that at times cause nerve issues.

      Seek medical assessment rather than continuing with treatment if you develop weakness in a limb, numbness that follows a nerve pattern and is worsening, or any change in bladder or bowel control.

    I have plantar fasciitis for 1 year in my left foot. WHat can be done about this?

      A year in, two things need saying, and the first is the one most often skipped.

      At twelve months this is no longer an inflammatory problem. The tissue changes seen in long-standing cases are degenerative rather than inflamed, which is why anti-inflammatory measures that helped early stop working, and why the treatment that works at this stage is loading rather than rest.

      The measure with the best support for persistent cases is high-load strength work for the fascia itself. The standard version is a heel raise performed with a rolled towel under the toes so they are held extended, which loads the fascia through the movement. It is done slowly, three seconds up and three seconds down with a pause at the top, on the affected leg, progressing from bodyweight to added load in a backpack, every second day. It is uncomfortable at first and it takes around three months to show its full effect, which is why people abandon it. Calf and fascia stretching, load management, and supportive footwear help alongside it, but the strengthening is the part that changes the tissue.

      Restricted ankle movement is the most consistently found contributor. Test it by kneeling with the toes about ten centimeters from a wall and driving the knee forward over the foot without lifting the heel. If you cannot reach the wall, that restriction is loading the fascia with every step and is worth addressing directly.

      For cases that have resisted a proper loading program, shockwave therapy has reasonable supporting evidence and is a fair next step. Steroid injection gives short-term relief but carries a recognized risk of fascia rupture and thinning of the heel fat pad, so it is worth understanding that trade before accepting one.

      The second thing, and it matters at a year: question the diagnosis. Several conditions are mistaken for plantar fasciitis and none of them respond to treating the fascia. The defining pattern of genuine plantar fasciitis is sharp pain with the first steps in the morning that eases within minutes and returns after prolonged standing, with tenderness at the inner front edge of the heel bone.

      If your pain is instead worse with impact and the heel hurts when squeezed from both sides, a stress fracture should be excluded. If it is burning or tingling, or if morning first-step pain is absent, nerve entrapment near the heel is a consideration. Pain centrally under the heel that is worse barefoot on hard floors suggests thinning of the fat pad rather than the fascia. Heel pain in a younger person, particularly if both heels are involved or other joints are affected, should prompt investigation for inflammatory causes. And heel symptoms can be referred from the S1 nerve root in the lower back.

      A year without progress is a reasonable point to have the diagnosis reconsidered rather than to continue treating the original label.
