What Flexion-Distraction Is Doing, and Why the Setting Matters
Flexion-distraction is a table-based technique. The lower section of the table moves, and the clinician guides flexion and traction through a chosen segment while feeling what that segment will accept. It is gentle by design, and it is not a machine you are simply left on.
The reason we use non-forceful, non-rotatory techniques on irritated discs is not preference. A systematic review of lumbar disc herniation treated conservatively, with repeat imaging in every included study, reported spontaneous regression rates of 96 percent for sequestration, 70 percent for extrusion, 41 percent for protrusion and 13 percent for disc bulging (Chiu CC, et al. In other words, disc material often reduces on its own if the segment is not repeatedly provoked. The job of care in that window is to keep the segment moving, unload it, calm the surrounding muscle, and avoid doing anything that sets it off again. That is the argument for controlled traction and against forceful rotation.
