GENERAL INFORMATION
Plain Guides to Back, Neck and Knee Pain
The guides below explain back pain, sciatica, slip disc, neck pain, knee pain and meniscus tears in plain language. Each covers common causes, why an assessment matters, and the non-surgical, conservative options people generally consider.
They are general information. They cannot diagnose you or tell you which option suits you. Only an in-person assessment by a qualified healthcare provider can find what is behind your symptoms.
BACK PAIN AND SCIATICA
Back Pain and Sciatica: Causes and Options
Back pain and sciatica can come from a spinal joint, a disc, or both. The pattern of your symptoms helps show which.
Back pain can make sitting, standing, bending and lifting harder. It can affect work, exercise and sleep. Some people feel stiffness more than pain.
Sciatica is pain that travels from the lower back or buttock into the leg. It can reach the thigh, calf, ankle or foot. It may feel like pain, tingling, numbness, burning, tightness or heaviness. Some people have sciatica with no back pain at all. Others have both. For some, the leg pain is worse than the back pain. For others, it is the other way around.
Other signs matter too. These include pain when you cough or sneeze. They also include pain when you bend at the waist, when you sit for a long time, or when you stand up after sitting. These signs can point to a spinal joint, a disc, or a mix of both.
Words like sciatica, pinched nerve and nerve compression are common. The symptoms alone do not show which structure is involved. Similar patterns can have different causes.
That is why it is worth finding the likely root cause through a thorough assessment by a qualified healthcare provider. It is also worth finding anything that adds to it. A thorough assessment covers your history and how the symptoms behave. It checks spinal and joint movement, muscle function and your daily loads. It checks nerve function when needed, and it takes any reports you bring into account.
Conservative options people generally consider for back pain and sciatica include chiropractic, physiotherapy and exercise therapy, alone or combined. Which suits a person depends on what the assessment finds. Some people need a medical, nerve or imaging review first, and a thorough assessment should tell you when.
What to Increase
Short walks through the day, standing up from your chair every so often, and the core and hip exercises you are shown.
What to Decrease
Long spells of sitting without a break, lifting while twisting, and heavy bending and lifting first thing in the morning.
SLIP DISC
Slip Disc: Causes and Options
Slip disc, also called slipped disc, is an everyday name for a spinal disc that bulges or pushes out beyond its normal edge. Which kind it is, and where, matters.
Slip disc covers several different disc problems. A bulge and a protrusion are not the same, and some discs push further, through the outer wall of the disc. They can happen in the neck, the upper back or the lower back. Some press on a nerve and some do not.
Many scan reports use words like spondylosis, disc degeneration, disc bulge or disc protrusion. These words alone do not show what is causing your symptoms. An assessment by a qualified healthcare provider connects the report to how you actually feel and move.
Some people like the feeling of a twisting movement, or of cracking their own back or neck. Something can feel good and still add strain. If you have a disc problem, ask any practitioner to explain a method, and why it suits your case, before it is used.
Conservative options people generally consider for a slip disc include chiropractic, physiotherapy and exercise. Some people need a medical review first, and a thorough assessment should tell you when.
If you already have MRI or X-ray reports, bring them to your assessment.
What to Increase
Walking in short, frequent spells, positions that ease your leg or arm symptoms, and the exercises you are shown.
What to Decrease
Bending and lifting at the same time, long sitting in soft, low chairs, and heavy lifting until your practitioner has reviewed you.
NECK PAIN
Neck Pain: Causes and Options
Neck pain can come from a joint, a disc or a pinched nerve. Each needs a different approach, so an assessment comes first.
Neck pain may come with stiffness, headaches, upper-back tightness, or pain into the shoulder and arm. Turning your head may be hard. Desk work, screens, driving and sleep position can all play a part. Numbness or tingling in the hand can mean a nerve is under pressure.
A thorough assessment by a qualified healthcare provider looks at how your neck and upper back move, and which segments are involved. It checks your shoulder position, muscle function, posture and work habits. It also checks whether a disc or a nerve may be part of the picture. An online description cannot find the cause. An in-person assessment can narrow it down.
Conservative options people generally consider for neck pain include chiropractic, physiotherapy, exercise and changes to posture and work habits. If a disc or nerve may be involved, ask any practitioner to explain a method, and why it suits your case, before it is used. Some people need a medical review first, and a thorough assessment should tell you when.
What to Increase
Regular breaks from screens, a screen raised to eye level, and the neck and upper-back exercises you are shown.
What to Decrease
Long periods looking down at a phone, sleeping on your front with your head turned, and cracking your own neck.
KNEE PAIN
Knee Pain: Causes and Options
Knee pain can start in the knee joint, behind the kneecap, or in the tissues around them. An assessment is how you find out which.
Knee pain can make walking, stairs, squatting, sitting, running and sleep harder. The knee may feel stiff, click, or not bend as far as it did. Three joints work together at the knee. One sits between the thigh bone and the shin bone (the tibiofemoral joint). One sits behind the kneecap (the patellofemoral joint). A small one sits on the outer side, between the two bones of the lower leg (the proximal tibiofibular joint). The muscles, ligaments and tendons around them matter too.
Some people have a report that says osteoarthritis, meniscus tear, ligament injury, joint degeneration or cartilage change. A word in a report does not show how much it adds to your pain today. It also does not decide the next step on its own.
A thorough assessment by a qualified healthcare provider looks at how the three joints line up and move together. It checks the knee, and also the hip, ankle, foot and lower back, because problems there can affect the knee. It watches how you walk and move, and asks how the pain changes with activity and rest.
Conservative options people generally consider for knee pain include chiropractic, physiotherapy and exercise, often with changes to daily activity. Physiotherapy may be hands-on, exercise-based or machine-based. Which suits a person depends on what the assessment finds. Some people need a medical or orthopedic review, and a thorough assessment should tell you when.
What to Increase
Strength in the thigh and hip muscles, regular low-impact movement such as walking or cycling, and the exercises you are shown.
What to Decrease
Deep squatting, kneeling for long periods, sudden twisting on a planted foot, and long runs on hard ground while the knee is sore.
MENISCUS
Meniscus Tear: Causes and Options
The meniscus is a C-shaped pad of cartilage between the thigh bone and the shin bone. Each knee has two. A tear can follow a sudden twist, or develop slowly with age.
A meniscus tear can cause pain along the inner or outer side of the knee, swelling, stiffness, or a catching feeling. Squatting, kneeling, twisting and stairs may be harder. Some people notice very little.
Some tears follow a twist on a planted foot, often during sport. Others develop slowly as the meniscus changes with age, sometimes with no clear injury.
Scan reports often use words like meniscus tear or meniscal degeneration. A tear on a scan does not always explain the pain you feel today. Meniscus changes are also common on the scans of people with no knee pain at all.
That is why a thorough assessment by a qualified healthcare provider matters. It looks at how the knee moves, where it hurts, how it behaves under load, and how the hip, ankle and foot affect it.
Conservative options people generally consider for a meniscus tear include physiotherapy, exercise and chiropractic, along with changes to daily activity. Some knees need a medical or orthopedic review first, for example when the knee locks, cannot fully straighten, or swells quickly after an injury. A thorough assessment should tell you when.
If you already have an MRI report, bring it to your assessment.
What to Increase
Gentle bending and straightening within comfort, thigh-muscle work while sitting or lying, and short walks on level ground.
What to Decrease
Deep squats, kneeling, pivoting on a planted foot, and carrying heavy loads up stairs while the knee is sore.
MORE GUIDES
Other Joint and Movement Concerns
Upper-limb problems can affect reaching, lifting, carrying, gripping, typing, exercise and sleep. They include shoulder stiffness, reduced movement, elbow pain, wrist strain and hand symptoms. Some involve more than one area of the neck and arm.
A thorough assessment looks at movement in the affected area and at muscle function. It also looks at posture, the neck and upper back, and how you use your arm at work, in exercise and in daily tasks.
Conservative options people generally consider include physiotherapy, exercise, rehabilitation and chiropractic, along with changes to activity. Care should never be chosen from the name of a symptom alone.
Ankle and foot problems can affect standing, balance, walking, running, stairs, footwear and exercise. Symptoms may involve the ankle joint, heel, arch, forefoot, toes, or the muscles and soft tissues around them.
A thorough assessment looks at ankle and foot movement, muscle function, balance and walking. It also looks at footwear, activity demands, and links with the knee, hip or lower back.
Conservative options people generally consider include physiotherapy, exercise, activity planning and rehabilitation. Foot or ankle symptoms do not always start in the foot or ankle.
Stiffness, pain or movement difficulty can be linked to sitting, standing, screens, driving, lifting, repeated tasks, exercise, sport or a change in activity.
Posture is only one part of an assessment. A thorough one also looks at movement habits, workload, strength, coordination, joint motion, recovery between activities, and how the problem changes during different tasks.
Conservative options people generally consider include chiropractic, physiotherapy, guided exercise, movement education, rehabilitation, and changes at work or in activity.
YOUR CARE PLAN
How Your Care Plan Is Made at CSC
Every case is assessed on its own. You get options, the reasons behind them, and the final say.
We start by listening. The interview covers your symptoms, your history, your work and your daily routine. Then comes the physical assessment. After that, we report what we found, in plain language.
Most people are then given two or three care options. We explain what each one offers and where it falls short. We tell you which one we would recommend and why. The choice is yours.
Options matter because every life is different. Time and cost can make a full plan hard to follow. A smaller plan you can keep to may serve you better than a larger one you cannot.
Nothing is forced. You can accept a plan, change it or decline it. There is one exception. We will not do something we believe could add strain to your condition, even if you ask. We will explain why and offer a safer option.
Recovery tends to go better when two things happen together. You increase what helps your condition, on your own or with help from a chiropractor, a physiotherapist or your doctor. At the same time, you decrease what harms it. For any care to help, the problem must first be assessed accurately. The plan must be sound and carried out well. Changes to posture, habits and daily activities complete it.
